Health History Form


For the past 16 years, we have had tried to expedite the dreaded medical office paperwork that all patients must complete by putting all the necessary forms online. In this way, our patients could download the pdf files, print them out and bring them to our office on their 1st visit.

For better or for worse, new government regulations (HITECH) require us to abandon the traditional paper medical chart and make the transition into a fully computerized electronic medical record by 2014. The transition has been all encompassing, involving every aspect of our practice, and somewhat painful. It certainly has made us much less efficient in some areas. There is a definite "learning curve." WE hope that this will improve over the next 12 months.

One thing that we had to eliminate, for the time being, was our downloadable patient medical history form. For now, we must ask our patients to arrive at least 30 min before an office visit and 60 minutes before a procedure requiring anesthesia. This allows extra time to fill out the necessary paperwork. Eventually, we plan to add a Web-based patient portal over which our patients will be able to complete all necessary registration or medical history forms online. For now, this is the best we can do.
